What is a medical home? 

 

According to the American Academy of Pediatrics: "A medical home is not a building, house, or hospital, but rather an approach to providing comprehensive primary care. A medical home is defined as primary care that is accessible, continuous, comprehensive, family centered, coordinated, compassionate, and culturally effective."

In addition to the daily services we provide for your child, we are also focusing on the chronic conditions of Asthma, ADHD and Health and Nutrition.  We have recently added a nursing care coordinator to help with the needs of our adolescent patients. 

Our office Care Coordinators will be working closely with our physicians to assist in ensuring that your child receives the best care possible.  With proper medical management, your child will live a happier, healthier life!
